Hello KHS Families,

If you have a child that is currently 16 or older, I would greatly appreciate you taking a minute to complete the following survey.  Our school district may possibly host an onsite vaccination clinic and I need your feedback.  By submitting this survey you are NOT registering your child for a vaccine. There is no guarantee we will be holding onsite vaccinations for students. This is just to determine if there is interest in having 16+ year old students vaccinated.
