Hello Kirtland High School Families,

We were just notified by the Lake County General Health District that Kirtland High School will be hosting a COVID-19 vaccination clinic on Wednesday, May 5th from12:00-2:00 (this is a Gold Day) for any interested Kirtland student that is age 16 and up. The second shot will be given on Wednesday, May 26th (Gold Day).

The attached permission form also needs to be filled out and brought with the student to the vaccination clinic on May 5th.
